I installed the User Forms module and it works wonderfully, no problem creating a new form. When I open the form on my website however, the form fields are all justified to the right, but the field names are to the left as they should be. I'm using the Blackcandy theme, I'm looking in the form.css file (themes/Blackcandy/css/) and there are a few changes I can make in there, but nothing that moves the form fields to be not justified to the right. Can anyone tell me where to look to change this? Thank you.

I seem to have a work-around - I placed the $UserDefinedForm inside of a table, and it made it look very nice, except for my radio button field, it looks like I need to make it wider, the text is underneath the buttons.

It was a theme issue. When I changed themes, the form displayed nicely. I had to edit the form.css file to change Labels to "display:inline" from "dislpay:block", now it looks very nice!
