multiple userdefined forms with different templates

We've built a site with 4 basic template types. I need to be able to utilise a userdefined form on each of the 4 templates, but the standard module just extends from page.ss. Is it possible to be able to choose which template to use it with or to allow multiple instances of userdefined forms ? The url is www.beachhaus.co.uk
