[Newsletter] Add extra fields

Hello Silverstripers,
I've been trying to add extra fields to the SubscriptionPage, with no luck.
So far I found that I need the fields 'Organisation' and 'Salutation' in the form, but what files do I have to edit to display those fields in frontend and backend?Thanks in advance
