Going through that tutorial was good and I was able to add all the fields I wanted, but is there a way to change the select boxes to radio buttons?

Also I need to create a form that has text in between fields, so it would look like:

First Name
-Field here
a bunch of text of about how we contact you so you know we don't spam you
Method of Contact
-radio buttons (2 or 3 options, could grow, but I know how to do that)
I don't care if this is hard or easy, I just need to be pointed in the right direction on where I can find to do this.

I ended up building my own custom form and used new SelectionGroup() for my radio buttons.
