Moved site, Userforms not working

I moved a site from one server to another, had to fix a few things and rebuild the database a few times and then everything seemed to be fine, until I looked at the Contact Us page, which was set up using the Userforms module.
The UserDefinedForm page type shows up fine, and all the tabs for the module show up in the CMS. Only the "Form" tab now shows up blank and the form doesn't show on the new site. What could I have done wrong when moving the site to cause this to happen?

Hi, just wondering if you ever solved this. i've got exactly the same problem.
i didn't 'move' the site though, just been setting up a site on subdomain of one of my other sites...
I just installed the userforms module, and like you say - the 'form' tab is just totally blank.i've checked the html source, and the tab is just an empty div...

solved::
although i have no idea why...
i uploaded files for the module through cpanel and exracted them on the server, instead of ftp'ing up. refresh cms: no change
then i added "Director::set_environment_type("dev");" to my config file. refresh cms: works!
so then i removed "Director::set_environment_type("dev");" from the config file, and it continued to work....

Hi - me back again... got the same problem again. last time i unzipped on the server, this time i don't have access to do that, so ftp'd up the files. going to dev mode didn't fix it this time... has anyone one been encountering this problem?
it doesn't seem to cuase any errors... just no content...
