UserDefinedForms - Email Recipient popup doesn't show any content

I've installed SilverStripe 2.4.7 on a clients german shared server (hoster: webhoster.de AG) with DOM, Uploadify, UserDefinedForm and have a problem with the userforms-extension 0.4 (look on attached screenshot). Email Recipient doesn't show after click on "add e-mail recipient".
On my other installations on another server/hoster/client is userforms okay.I have tested another fresh installation of SS 2.4.7 on the same server with another subdomain with userforms 0.5 – the same problem… -.-
I flushed the admin cache with /admin?flush=1 and /admin?flush=all – doesn't help… -.-does anybody know how to resolve it?
