upload file feature on userform

Hi everyone,
Received an error when trying to upload a file using the userform funtionality. I'm using SS 2.3.1 with userforms-v0.1.0 please see attachment for error. This is what I receive once I submit the form on my contact page.
Any help would be greatly appreciated.
