UserDefinedForms sends blank email

I've successfully managed to upgrade to 2.3.3 and am running the newest release of UserDefinedForms.
I can add an e-mail recipient, and it will successfully send an e-mail to the correct address, but the e-mail contains no data from the form. What am I doing wrong? -

Am also getting this in place of "Add Email Recipient":
<% sprintf(_t('ADDITEM', 'Add %s', PR_MEDIUM, 'Add [name]'),A Email Recipient) %>
