Howto create a newsletter subscription form?

I've created the newsletter, and am trying to make the subscription form. I assume I need fields "Firstname", "Sirname" (both text) and "email" (email) in the form, but when I build the page, and try to submit to it, I get the page again, with no submission. Running 2.2.0r1. Any ideas?
