Login Fields Disappear and Reappear and Disappear

I go to my admin screen and I get the following message (see attached image). It has always worked fine. I didn't change anything.
The login fields magically reappeared last night and I was able to get back into the CMS, however, it was too late to do any work in it. I tried to get in again in the morning and the fields were gone again.
Any ideas?

Make sure $Form is included in your template, you have cleared your cache (silverstripe and browser) and also do a flush.
