DynamicForm saving 3 inputs

Hello
I have very strange error, when I'm trying to build dynamic form with big amount of fields
I'm getting strange error.
Let say I have just added 5 Text fields and I'm saving it. When I'm looking in frontend on saved form I see only 3 first inputs. After refreshing under admin interface I see 3 fields too....
What is going on? -

Is that when creating forms through the cms?
I've had about the same weird error on some occasions. I always saw only the last (or first, can't remember) 3 fields, and never all of them - as if they weren't saved. I never figured out why this happened, as I didn't get an error either. I only got this on some remote hosts, while never on my localhost.
I ended up creating my forms manually instead of through the cms.

Yes exactly the same behavior ....
Anybody has some idea?
