Help needed recreating a form

Hi,
I am currently re-designing a boat show website. It is currently aspx and am rebuilding it in Silverstripe.
However, with UserDefinedForms i cannot see how/have the knowledge to customise the layout such as the tickboxes in the attachment. Everything seems very ummmm - vertical.
And there seems to be that tick box niggle where the text and box do not sit nicely.

Is there anyway that I could create the form in Dreamweaver and then create a page type and put the form into the template? But then there would be the problem of running php inside the template.

Can anybody point me in the right direction

Knowledge of php is unfortunately small : ( but will be doing a php project for my major at uni in October so hopefully I will get a better understanding/ability to write code. Any good hands-on books to learn php?
