Something I've just encountered:
If you have UserDefinedForm on several pages and only want to change the layout of one page then you will have to duplicate UserDefinedForm.php (found in cms/code/pagetypes) and .ss and rename them appropriately (ie ContactForm.php , ContactForm.ss). Make sure that the new UserDefinedForm.php has anything with UserDefinedForm replaced with ContactForm - so a simple Find and Replace should work, or do it manually.
