User Defined Form: Won't let me create form fields, throws out blank page

Hi, there. I'm new to Silverstripe and am trying to get used to it. I have successfully installed the User Defined Forms module and it appears in my Contact page editing area since I defined that page as a User Defined Form. However, when I try to add a form field and hit "Add", I get a blank page. Any ideas why this might be happening? Do you need my server specs?

I've set up a sandbox here:

UPDATE: I tried using this module in Opera and it did let me Add the form fields and I could see the message "saved" at the lower left of the screen. I then published the page and still nothing. In Opera, it goes one better than Firefox, by allowing me to add fields, But where are they going? There's something preventing the form fields from appearing, either to be edited or displayed on the published page. Surely this isn't a browser issue. I am thinking it's a server issue, but what?

Are you using the latest version of SS and userforms? You might also want to try the 'trunk' or 'daily build' of userforms. You can find this second download link on the userforms module page on the right hand side. After you download and install the trunk version run dev/build and reload the admin with ?flush=1 and see if you can add from then

Well I downloaded both Silverstripe and the User Defined Forms module last night and installed them this morning. So that can't be the problem.

I'm at my second job right now and haven't touched the site; it was working before I left the house. Now I get this only on the Forum link:

And this on the Blog link:

Is my web host at fault? Sounds like they're messing with server settings or something.

I'm afraid this non-functioning UDF module is a deal breaker. Anyone know any other way to incorporate forms into SS? Third party forms, hand coded forms?

I just can't get the &$%&#^^ thing to work, I've tried everything, every browser, reinstalling, rebuilding, everything. Should NOT be this problematic.

Hi!

UDF is working well here, and I have a lot of different forms working together nicely. Just a shot in the dark (I’ve seen several similar posts recently): ensure that your UDF folder is called ‘userforms’ or the needed JS won’t load.

OMG, Juan, I LOVE you. That was IT! Problem solved. Incidentally, the documentation for some of these modules is abysmal. Here's the exact readme file from UDF:

"Installation Instructions
-----------------------------------------------
Extract the userforms folder into the top level of your site, and visit
http://www.mysite.com/dev/build to rebuild the database."

Not once did it mention renaming the folder from its default "userforms-v.0.2.0"

P.S. Guess I should rename all my module folders to strip out versioning?

It’s a pleasure to help.

The readme file is indeed not clear enough, even if it says “extract the userforms folder…” (maybe something to improve, Will). Nevertheless, should you have opened the _config.php file, you would have find the solution in seconds. It is always a good idea to look into those files: you’ll always learn something about how modules work (and yes, remove version of the names of all modules you could have installed).
