I faffed around with the Apache setting yesterday and got it working. Although I wish I knew what it was that fixed the issue.

At a guess it would of been setting up a VirtualHost with the domain explicitly set as ServerName.

I've run into this issue now as well. Will start looking at some of the suggestions being made above.

UPDATE: I've looked into many of the suggestions above, but no dice. I've added a question on StackOverflow (https://stackoverflow.com/questions/44956081/directorprotocolandhost-lacks-sufficient-information-http-host-not-set).
