Not able to create new page

Hi,

I have created a new page type which uses HasOneComplexTableField. However when I try to create a page in CMS for this page type, I get a blank message box which continue to load the page but never gets created. The blank message box is attached.

Blank dialogue box and two PHP files being used are attached.

Any suggestion what am I doing wrong.
