Image won't insert into page from CMS Files

Sometimes when inserting an image into a page from the CMS, when SAVE is clicked, it crashes out of that page and goes to the HomePage in the CMS. However, if we upload an image directly from the page, it works. The issue only applies to some images. We have tried very small file sizes to ensure its not a server limitation.

Our client wants to upload images to the FILES section as they often re-use images across the site.

Any ideas as to why this happens?

I've seen pages crash, but never one that made the CMS revert to the Homepage... So just some thoughts:

What version of SilverStripe?
Does this happen on a local install?
So if it only happens to some images, does it then happen to those images always, so that you can recreate?
If so,did you place the site in dev mode to get some error messages going?
Is it the images themselves or the folder they're in?
Could it be a browser issue?
Can you find anything in the logfiles?
(If it happens in FireFox for instance, then FireBug might tell you something)

Version 3.1
We don't have a local install
It always happens to a specific image, on different computers and different browsers
Not tried dev mode - will do so now.
It's not the folder as other images in the same folder work.

Oddly, if I upload an image directly, save, then upload the same image but via the CMS, that works!
I'll try some of the debugging suggestions and report back.

If it would help, you could send me (a link to) the offending image, so I could try and reproduce...
