commerce coupon module [User Error] CouponForm form created without a controller

Hi,
I'm trying to install the commerce coupon module and everything looks to be working ok a part from when I go in to the checkout page I get the following error:
[User Error] CouponForm form created without a controller
GET /checkoutLine 164 in /var/www/projects/readyornot.co.nz-dev/sapphire/forms/Form.php

Hi Roger
I would really like to help you with this problem as I wrote the coupon module ( well, at least one of them).
can you tell me what version of e-commerce you are using?
The coupon thing is working here: www.silverstripe-ecommerce.com so in theory it should work ;-)
Any other feedback on the module would also be appreciated.

PS you probably want to get a calling stack to see what calls what in ending up with your error.
