Inserting Contact us Form on Live Website

I am a newbie in Silverstripe. Please I need help on how to insert a Contact us form on a live website. I do not seem to get the right directory when i followed this tutorial: http://www.ssbits.com/newbies/2010/creating-a-simple-contact-form/
The form was not created
Any help will be appreciated -

use userforms and create the form in backend
