Form Action to External Third Party Source

I am using Silverstripe to build the form but I need to point the form action to the external third party URL to do the form processing.
Can someone point me in the right direction to set this form action to this external source? -

After the results have been processed, does it need to post back the results on your page or do you only need the form to display?
If the latter, you could easily just add in the form html with the correct action in your templates.
