Call userdefined form on home page

Hi, Tom Here.......
I went through many forums but i cldnt find a technique which opens the form in a popup window. Finally I got a script But it only works in userdefined page . Wht i need now is, i want to call my form on home page..............
Is it possible to call user defined form on home page or any other page ???
