userforms - did a build, what next?

I ran the build after copying across the userforms folder. The build appeared to run correctly however when I log in I cant see any option to add a form to my page?
What do I need to do next?

Hi Anthony
You need to add a 'UserDefinedForm' page to your site. You can't add a form to any page.

So I did that. I changed 'Contact Us' to be a userforms page.
However when I go to the 'form' tab, I see a duplicated list of forms. The list is also vertical, not horizontal like I have seen in the screenshots of the userforms module so I think something is wrong.
Clicking on the options also does nothing.
Thoughts?
