Hi!
I have downloaded the Form module here:

I installed it as described and did run "../dev/build".
Now, I can change the page`s behaviour to "User Defined Form" - but unfortunately - I cannot use at all its possibilities - see attachment.
Has this something to do with restrictions or settings from my web server?

Did you name your module directory "userforms" ?

I am having this same problem, have the file named correctly userforms, however I notice the form that I downloaded tells me that it will work with v 2.3.2 I am using 2.3.3
