ImageField from File Store deletes connection to image on other page.

Hi,
my client tries to add an existing image from the file store through the ImageField. He is experiencing a strange eroor, though. The image is no longer linked to the original page it was uploaded to.
What makes it extra difficult is the fact that I can’t seem to reproduce it. I have seen some strange behavior with File Store before, though. Anyone have any idea where I could start to look?
