Missing Passwords

Hey,
I have a few sites, where the passwords just stop working. I have read about different versions of php causing this issue, but it doesn't look like php has been upgraded.
It just happened to a site running 2.3.4 but another site on the same server running 2.3.3 was unaffected.
Anyone seen anything like this before?
