[Form] using specific templates with forms (extends Form)

Hi,
I want to use a specific template when I create a form. I read this tutorial and this one.
So, how can I create my own template (.ss file) ?
In the second tutorial, i try to use my own class Form like the example in the "Using a custon template" section. But for now, how can I use it ?, and where ?, in my class Contact_Controller extends Page_Controller { file ?
I don't know how to do.
Thanks,
Fabrice
