[Solved] Haven't changed anything after installing userform module

The CMS requires that you have JavaScript enabled.

Ensure that your web browser has javascript. This is web browser specific and not SilverStripe related. Eg for Firefox - http://mistered.us/tips/javascript/firefox.shtml
