UserForms 0.1 Upgrade from SS 2.2.1

Hello All,

I am upgrading a site from SS 2.2.1 to SS 2.3.2. The I see that the native UserDefinedForms are now in the UserForms module.
I have the module installed and can reset old Form pages to User Defined Page type after the upgrade. The existing forms are then displayed in the form tab for the page. I can also see all of the previous submissions for this form page.

However, when I navigate to the page the form is not displayed on the website. What have I missed? My Page.ss file contains the $Form variable as it did prior to the the upgrade.

Any ideas or links to relevant posts?
