Yup. Don't know how much of a newbie you are.
You download the userforms module. It'll be on the Extensions (you can see it in the black strip on top of this page)
Put the folder in with the rest of your silverstripe stuff
Rename the folder to "userforms" (get rid of the version id and stuff like that)
Run /dev/build from your browser
If you go into the CMS and create a new page type, you will see "user defined form" among your choices.
Create a page of that type.
The page will have a bunch of tabs, choose the "Form" tab
Pick the field types you want from the list
Save and publish
Bob's your uncle. Pretty much.
The page will still have a $Content variable. Your next step is to edit the tamplate (page.ss) to have two divs, with the $Content on one side and the form on the other. Or something a lot like that.
