User Defined Form - not working

Hi!

I have downloaded the Form module here:
http://doc.silverstripe.com/doku.php?id=modules:userforms&s=user%20defined%20form#download

I installed it as described and did run "../dev/build".

Now, I can change the page`s behaviour to "User Defined Form" - but unfortunately - I cannot use at all its possibilities - see attachment.

Has this something to do with restrictions or settings from my web server?

this looks like Silverstripe can't find some files.

Did you name your module directory "userforms" ?

I am having this same problem, have the file named correctly userforms, however I notice the form that I downloaded tells me that it will work with v 2.3.2 I am using 2.3.3
