I have implemented SilverStripe as a CMS over the last couple of days for a client. During this time, I had never logged out of the CMS.
Now after logging out and trying to log back in it takes me to a login screen that says:

Please choose an authentication method and enter your credentials to access the CMS.

But there is no form for me to enter a username and password.

Whats going on?

Have you tried going to...

www.yoursite.com/Security/login

?

Yes, when I goto http://localhost/mysite/index.php/Security/login

It returns a screen that says Login.

But there is no form to enter my credentials.

Probably you have removed from your page template the $Form variable, which is required to render forms.

hahaha good call... I may have removed it during other troubleshooting, I will check when I get back to my workstation.
