UserForms SS3 - Form Fields?

Using SS3, So I downloaded the UserForms zip from GitHub which works but doesn't allow me to add any form fields....
How can I resolve this? -

Ensure you downloaded the latest master version (not the last stable release) and after installation make sure you use flush=all when accessing the admin panel to clear the cached templates
yoursite.com/admin?flush=all

I've the same problem, i remove the stable and install the master version but now all I have is a forbidden when I try to add a new "User Defined Form" page.
What's wrong?

"yoursite.com/admin?flush=all" worked for me with out changing the module version I got from github.

Hi there,
Any more info on this?
I've installed the master version and run a ?flush=all after dev build, and although I can add a form page, there is no form builder visible in the Form tab.
